Why Use ePlan?
For outstanding care planning:
e-plan supports outcome based care planning to achieve the aspirations, goals and priorities identified by service users. Our care plans are to be created with the service user/decision maker to establish areas of need, goals/outcomes and the support required to achieve the desired outcomes, identifying the service user’s preferences and priorities. Assessment tools and risk assessments are included to identify and record needs/risk.
For creating care actions:
Care actions can be created after each care plan is completed. They act as a quick reference to brief home care staff on how to meet the service user’s desired outcomes. Care actions ensure that day to day support is given and reported on in a timely manner in line with Key Lines of Enquiry (KLOEs), and that desired outcomes are being met. The actions act as evidence to show the care that is being provided.
Reports and audits:
Quick access reporting and auditing on care given, reviews, assessments outstanding etc. to assist management in governance, ensuring processes are in place to achieve outstanding levels of care.
Easy to use:
A system that is easy to navigate, intuitive and user friendly, irrespective of tablet/device experience.
Cloud based, secure server.
All staff have unique log in/password.
Specific and confidential access can be given to professionals such as CQC Inspectors and Social Workers to demonstrate that full and appropriate support is being given to the service user(s) e.g. care planning, reviews, reports etc. e-plan enables you to evidence that the support you are giving is safe, effective, caring, responsive and well led, in line with the Care Quality Commission Key Lines of Enquiry (KLOEs).
